CHADIS
Dear Patients,
We are pleased to now have a new method to help us better identify any problems with your child's development and behavior. This is called Chadis (Child Health & Development Interactive System.) Please follow these instructions to send the information to us.
INSTRUCTIONS
1. Go to CHADIS
2. Go to 'New' or 'Returning' user on the left side bar to log-in
3. Enter our practice "invitation code" which is our phone number 3018692292
4. Enter your email address and make up a password (if not already done)
5. Enter your child's information and select your child's provider
6. Choose between developmental/emotions/behavior visit (new or follow-up) and regular (or well-child) visit
7 Your email address, if you have one, is the best username. if you do not have email, you can still register and use Chadis
Your child's doctor or practitioner will review the information you entered and discuss your results with you. This input is essential in helping us to provide your child with the best care possible. We appreciate your time, and look forward to seeing you and your child soon.
